Zusammenfassung

Advances in surgical techniques and improved medical therapies, specially related to immunosuppression, have decreased the mortality of liver transplantation (LT). The development of metabolic syndrome (MS) in these patients is a determining factor in medium and long term mortality caused by cardiovascular diseases. The onset of MS is not caused only by the transplantation but also by graft factors and associated diseases of the donor. Several studies have described the relationship between cardiovascular diseases and donor´s SM features. A special mention goes to patients transplanted for viral hepatitis C (VHC). In those patients the recurrence of VHC infection in the graft is considered universal, it occurs earlier, aggressively and with poorer response to antiviral therapy in patients with MS , in which the insulin resistance plays a key role. Studies to clarify prevalence of MS in these patients, risk factors and the possible relationship with the severity of HCV recurrence in the graft are required to implement preventive measures and improve prognosis in this population.
