Resum

Dader Method for pharmacotherapy follow-up was designed by Research Group on Pharmaceutical Care at University of Granada, and it is based on obtaining patient�s pharmacotherapy record, this is to say health problems patient suffers and medicines he or she uses. Then this situational sheet is assessed in a definite data, in order to identify and resolve pharmacotherapy failures (PTF) patient may be suffering. After this identification, pharmacist necessary interventions are done to resolve those PTF, and outcomes are assessed. To implement Dader Method to inhospital patients a number of adaptations to the stages in this method are to be done., because of it was designed for community pharmacy, where differences appear with the hospital environment. This work tries to describe adaptations to be done in Dader Method for pharmacotherapy follow-up in order to implement on caring inhospital patients, so as to be able to probe the role of hospital pharmacist in caring process to patients from the admission to discharge.
