Abstract

Second consensus of Granada on Drug-related problems (DRP) define them as health problems, understood as negative clinical outcomes, resulting from pharmacotherapy, that produced by different causes, lead to not achieving the therapeutic objective or to the appearance of undesirable effects. In this consensus, WONCA definition of health problem as �any compliment, observation or fact that patient and or physician recognize as a deviation from normal that affected, can affect or affects patient functional capability�. The objective of this paper is to define DRP and risk of DRP after the definition of WONCA�s definition of health problem, and to establish criteria to determine what can be considered as risk of DRP and the tools available in pharmacotherapy follow-up to detect them.
