Zusammenfassung
The identification of every negative clinical outcome from pharmacotherapy is made easier if it is made through a systematic evaluation process. There have been designed several algorithms to carry out/accomplish this process. In 1999, the first adapted algorithm to the Consensus of Granada about Drug Therapy Problems was designed. The accomplishment of the Second Granada Consensus on Drug Therapy Problems, the design of a more simple model of assessment form and the accumulated experience after five years, recommend to update the text of that first identification system.
