Abstract

Fracture-dislocationsofthelittlefingercarpometacarpal(CMC)jointarecommonlymissed.Delayed diagnosis and treatment will usually result in an undesirable outcome of pain,reduced grip strength, and degenerative arthritis. Treatment of the old dislocations of thefifth carpometacarpal joint is challenging. Numerous surgical techniques have beenproposed. Here, we present a modification of a CMC joint suspensionplasty, using apartial slip of theExtensor Carpi Ulnaristendon. We believe this technique has severaladvantages compared with other palliative surgical procedures. First, mobility of thefifthmetacarpal is greatly preserved because thefifth metacarpal is stabilized only by theligamentsubstituteagainst thefourthmetacarpal.Second,thelengthof thefifthraycanbemaintainedeffectively.Third,malrotationofthelittlefingercanbepreventedbydrillingtheholes of thefifth and fourth metacarpals in the proper direction. We believe this surgicaltechnique could be a better treatment option for old dislocation of thefifth CMC joint
