Abstract

Introduction. Carpal tunnel syndrome es the most common entrapment neuropathy of the hand. Articles com-paring postoperative clinical results after carpal tunnel release, with or without flexor retinaculum reconstruction, get very different conclusions. The aim of this work is to carry out a review of the bibliography to reach conclusions based on the evidence. Material and method. A systematic review of scientific articles from the MEDLINE database was carried out. Articles that compared simple opening with the reconstruction of the carpal flexor retinaculum in patients with idiopatic carpal tunnel that describe reconstruction or elongation techniques of the flexor retinaculum were considered. More articles of bibliographic references were obtained. Results. 17 articles from MEDLINE and 4 from bibliographic references were selected. Among the articles of greatest interest we highlight 7 clinical trials and a metaanalysis. Conclusions. With the available evidence we can not affirm that the release of the carpal tunnel with reconstrucion of the flexor retinaculum is superior to the simple release in terms of grip and clamp force, improvement of the symptoms of compression of the median nerve or in the functional recovery of the hand.
