Introduction:Perineal eventration is a rare complication of abdominoperineal resection of the rectum and rarely requires surgical treatment. Clinical case: A 60-year-old man with strangulated perineal eventration causing intestinal obstruction. Urgent surgery is performed: mechanical resection of the affected ileum loop and confection of isoperistaltic manual laterolateral anastomosis and placement of a Symbotex composite mesh for the closure of the minor pelvis. Discussion: Due to its low incidence, surgical treatment is not standardized. This pathology should be taken into account in the case of a patient with a small bowel obstruction and a history of proctectomy.
