Pasado y presente del tratamiento frente a la COVID-19

Resumen

Desde el inicio de la pandemia por COVID-19 en diciembre de 2019 se han ido proponiendo y utilizando numerosos fármacos en un intento de hacer frente a esta nueva enfermedad, con frecuentes cambios y actualizaciones en las guías de tratamiento y sus recomendaciones. Así pues, este trabajo se plantea con el objetivo de mostrar cómo ha sido la evolución del tratamiento de la COVID-19 a lo largo de este periodo. Para ello, se llevó a cabo una revisión de la literatura disponible en Pubmed, MedRxiv y las guías de tratamiento WHO, NIH y NICE. En base a la información obtenida de estas fuentes se concluye que de todos los medicamentos propuestos, tan solo los corticoides, Remdesivir y Tocilizumab han mostrado algún beneficio en los estudios realizados en pacientes con COVID-19; mientras que Cloroquina, Hidroxicloroquina y Lopinavir/ritonavir, fármacos muy utilizados al inicio, han quedado totalmente descartados. Por su parte, Anakinra, Baricitinib y Colchicina han mostrado un potencial beneficio en los primeros estudios, aunque todavía no se tiene una clara evidencia de su eficacia que permita su recomendación como tratamiento en este tipo de pacientes. En cuanto a la anticoagulación, no hay evidencia que justifique el uso de dosis terapéuticas o intermedias en pacientes hospitalizados con COVID-19, por lo que a día de hoy se recomienda la administración de dosis profilácticas
