Resumen

Resumen Las lesiones mitrales constituyen una de las partes fundamentales de las enfermedades cardíacas. Se clasifican en base a su disfunción en estenosis e insuficiencias. La estenosis mitral de etiología reumática, todavía frecuente en países en vías de desarrollo, está siendo sustituida por la de origen degenerativo, cuyo diagnóstico y manejo difiere del de las reumáticas. En la insuficiencia mitral hay que distinguir dos entidades totalmente distintas: las insuficiencias mitrales primarias u orgánicas, en las que el aparato valvular está afectado, y las insuficiencias mitrales secundarias o funcionales en las que la válvula es normal pero se produce insuficiencia porque la disfunción ventricular impide el adecuado cierre de la válvula. El tratamiento de las lesiones mitrales ha cambiado notablemente en los últimos años. El habitual recambio valvular por una prótesis ha sido sustituido por técnicas de reparación valvular. Las técnicas de intervencionismo percutáneo están plenamente establecidas en la estenosis mitral reumática y cobran cada vez más protagonismo en la insuficiencia mitral para casos con alto riesgo quirúrgico.
