Abstract

Background. Non-invasive assisted ventilation (NIV) is a technique of ventilatory support widely used in adults, and wich main benefit is the avoidance of the adverse effects related to endotracheal intubation and mechanical ventilation. Objective. The aim of this report is to demonstrate the benefits of using NIV in children, in particular the use of bilevel intermittent positive airway (BIPAP) during sleeping time. Methods. This is a descriptive report on 4 children with neuromuscular disease who developed hypercapnic respiratory failure. NVI-BIPAP was administered with a nasal mask and the use of a BIPAP-S/TD30 ventilator (Respironics®), wich delivers two levels of pressure synchronized with the respiratory movements of the patient. The NIV-BIPAP was applied during sleeping hours, and the amount of time was increased by 2-hour aliquots according to the patient’s tolerance. Results. Clinical, radiological, and pulsioximetric improvement was observed with the use of NVI-BIPAP in the four cases. The domiciliary nocturnal administration of this assisted ventilatory technique has reduced the number and severity of respiratory complications in this group of patients. Conclusions. The use of NIV-BIPAP in the hospital setting allows for the avoidance of endotracheal intubation in patients with neuromuscular diseases. The use of NVIBIPAP at home reduces the respiratory morbidity of these patients, improving their quality of life significantly
